Attending Physician Statement (APS) (APS)

An attending physician statement is a report or set of medical records that a carrier requests from your doctor so it can review your health history during underwriting.

Why it matters

Carriers order an APS when answers show a condition that needs detail, or for larger policies. It can slow approval by weeks. Applicants sign a release so doctors can share records. Records must be accurate and complete. Many simplified plans avoid the APS, but the carrier may still ask for one.
